Commitment to act as a member of the network (required)

I hereby make commitment to act as a registered member and an expert (of my own field of the specialty) in NordicPedMed / FinPedMed network as defined in the network terms. I understand that the operating model of the network requires the YES or NO response to these Service Requests, and I commit to reply to these requests as soon as possible after I receive these by e-mail.

Statement of confidentiality (required)

I hereby understand that all Service Request related documents (i.e. the request information and the attachments) are confidential and must be treated as confidential information according to the terms of NordicPedMed / FinPedMed network, and agree to act according to these terms; not re-sending, copying, photographing, recording or otherwise revealing the request information to third parties outside the network members / experts.

Consent to record and save this given information to network registry (required)

I hereby accept that my personal data of this NordicPedMed/FinPedMed registry can be saved, handled and further managed according to the network terms in order to manage e-service -model for Service Requests, and national regulations of handling the personal data (national Personal Data Act or Ombudsman registry notification rules).
